ABSTRACT

In this study, we aimed to analyze the effect of 5-fluorouracil, triamcinolone, and bevacizumab on scar modulation in an experimental rat model of surgical lesions. Rats (Rattus norvegicus albinus) were divided into four groups: bevacizumab, 5-fluorouracil + triamcinolone, bevacizumab + 5-fluorouracil + triamcinolone, and control (received no medication) groups. A linear, dorsal incision was created and sutured for the first intention wound healing, mimicking the surgical incision of upper blepharoplasty. Treatments were initiated on day 7, and the rats were euthanized on day 14. Only in the 5-fluorouracil + triamcinolone group was there a difference in the number of infiltrated monocytes. There was 56%, 86%, and 85% decrease in the number of neovessels in the bevacizumab, 5-fluorouracil + triamcinolone, and bevacizumab + 5-fluorouracil + triamcinolone groups, respectively, compared with the control. Picrosirius red staining showed higher collagen density and more organized collagen in the treatment groups than in the control group. Scar modulation was observed in all groups, but the 5-fluorouracil + triamcinolone group presented the best results. To our knowledge, this is the first study to evaluate the influence of three medications in combination on healing. When used together, these medications can prevent the development of unsightly scars, and are therefore promising alternatives to corticosteroids.

Introduction: The relationship between obesity and physical activity is still unknown in specific groups, such as people living with ostomies. Objective: To assess the prevalence of obesity and the level of physical activity in ostomized individuals. Methods: A cross-sectional study with a population of ostomized patients in two hospitals of a Brazilian city. Obesity was assessed by the Body Mass Index (BMI) and physical activity, by the International Physical Activity Questionnaire. Results: The study included 148 patients (55.4% of men [82]), with a mean age of 58.5 (±17.8) years, with 56.1% (83) of the sample aged 60 or older. Colostomies were identified as definitive in 67.6% (100), there were 28 cases (18.9%) of obesity, 67 (45.3%) overweight patients, and 31 (20.9%) subjects were underweight. Sedentary lifestyle was identified in 40.5% (60) of the participants. The mean BMI was significantly higher in men (25.9 ± 5.1), and a higher proportion of men were overweight (28; 34.1%); the underweight group had a greater proportion of women (20; 30.3%). An association between physical inactivity and underweight (p = 0.003) was found. Conclusion: Most patients with ostomies, especially colostomies, are men over 60 years of age who are overweight or obese. In general, underweight individuals are more sedentary. (AU)

ABSTRACT

Chromomycosis is a fungal infection that affects the epidermis, dermis, and subcutaneous tissue and is caused by dematiaceous fungal species that turn black on staining. We report the case of a 50-year-old male patient who was a rural worker and had been treated without success for three decades. Facial lesions progressed and caused severe cicatricial retraction. As the infection evolved, the left upper eyelid developed cicatricial ectropion. The surgical treatment was performed using skin obtained from the patient's own abdomen. Patient has developed a good postoperative appearance.

RESUMO A cromomicose é uma infecção fúngica que afeta a epiderme, derme e tecido subcutâneo. A infecção é causada por espécies de fungo dematiáceos que se coram em preto. Nós relatamos o caso de um homen de 50 anos de idade, trabalhador da zona rural, que tinha sido tratado por três décadas sem êxito conclusivo. As lesões faciais progrediram causando retração cicatricial severa. Com a evolução do quadro, houve também retração também da pálpebra superior do olho esquerdo. O tratamento cirúrgico foi realizado utilizando pele abdominal do próprio paciente. O paciente apresentou uma boa aparência pós-operatória.

ABSTRACT

Adequate management of phyllodes tumors of the breast (PTB) remains a challenge because of the difficulty in correctly establishing preoperative diagnosis. The aim of this study was to evaluate the usefulness of Ki-67, CD10, CD34, p53, CD117, and of the number of mast cells in the differential diagnosis of benign PTB and cellular fibroadenomas (CFs) as well as in the grading of PTB. Fifty-one primary PTB and 14 CFs were examined by immunohistochemistry.When evaluating CD117 expression, higher epithelial expression was present in CF as well as an increased number of mast cells in benign PTB. Stromal expression of Ki-67, CD10, CD34, and p53 were relevant to PTB grading, of which the first 3 showed significance in the distinction of benign and borderline PTB, as well as between benign and malignant PTB. P53 was relevant only for the discrimination between benign and malign PTB. None of the markers showed significance in distinguishing between borderline and malign PTB.

BACKGROUND: Medical records are frequently consulted to verify whether the treatment and guiding principles were correct. Determine incidence and mortality trends of in situ and invasive neoplasms of the uterine cervix, in the period 1988-2004 in Goiânia, Brazil. METHODS: The incident cases were identified through the Population-Based Cancer Registry of Goiânia. Population data were collected from census data of the Brazilian Institute of Geography and Statistics. For mortality analysis, data were extracted from the Mortality Information System. The Poisson Regression was utilized to determine the annual incidence and mortality rates. RESULTS: A total of 4446 cases of in situ and invasive neoplasms of the uterine cervix were identified. No significant reductions were verified in invasive cervical cancer rates (p=0.386) during the study period, while in situ carcinomas presented an annual increasing trend of 13.08% (p<0.001). A decreasing trend was observed for mortality (3.02%, p=0.017). CONCLUSION: No reduction was observed for the incidence of invasive cancer of the uterine cervix; however, increasing trends were verified for in situ lesions with a consequent reduction in mortality rates. These increasing trends may be the result of recently-implemented screening programs or due to improvements in the notification system.

ABSTRACT

O antígeno Ki-67 é utilizado para avaliar a atividade proliferativa em vários tumores. No entanto, o papel do Ki-67 como fator prognóstico em câncer de mama é ainda indefinido. O objetivo do trabalho foi analisar a expressão do antígeno Ki-67 correlacionando com fatores clínico-patológicos e sobrevida em pacientes com câncer de mama. Realizou-se a análise imunoistoquímica utilizando anticorpos monoclonais MIB-1 em 140 amostras de câncer de mama de mulheres com idade entre 27 e 90 anos, atendidas no Hospital Araújo Jorge da Associação de Combate ao Câncer de Goiás. O ponto de corte para a análise foi de 25%. A sobrevida global em cinco anos foi de 77,1%. Houve sobrevida significativamente pior nos casos com linfonodos positivos (p < 0,0001), tumores maiores (p < 0,0001), estádios mais avançados (p < 0,0001) e pacientes com receptores hormonais negativos (p = 0,019). Pacientes com imunodetecção de Ki-67 em mais de 25% das células tiveram pior sobrevida quando comparadas àquelas que expressaram Ki-67 em menos de 25% das células examinadas (70,8% x 82,7%), porém essa diferença não foi estatisticamente significativa (p = 0,094). A hiperexpressão do Ki-67 não se correlacionou com pior sobrevida no grupo de pacientes analisadas.


The Ki-67 antigen is used to evaluate proliferative activity in several tumors. However the role of the Ki-67 as a prognostic factor in breast cancer is still undefined. The aim of present study was to analyze the expression of Ki-67 antigen correlating with clinicopathological factors and survival in patients with breast cancer. We carried out an immunohistochemical analysis using MIB-1 monoclonal antibody in 140 specimen of breast cancer of women with age between 27 and 90 years. The cut-off point was 25%. The five year overall survival was 77.1%. There was a significant worse survival of patients with node positive (p < 0.0001), larger tumors (p < 0.0001), more advanced stages (p < 0.0001) and with negative hormonal receptors (p = 0.019). Patients with Ki-67 immunodetection in more than 25% of the tumors cells showed a poorer outcome when compared with those that express Ki-67 in less than 25% of the cells examined (70.8% x 82.7%), however this difference was not statistically significative (p = 0,094). The overexpression of Ki-67 did not correlate with a worse survival in the group of patients analyzed.
